HICAP is a volunteer-based program that benefits older adults and persons with disabilities. The goal of this program is to provide objective counseling, advocacy, and assistance with Medicare, health insurance, and related health coverage plans for Medicare beneficiaries, and to educate the public on Medicare and health insurance issues.

PURPOSE OF WORK: To provide administration, leadership, and supervision of the HICAP office operations. Implement policies and procedures as assigned by Program Manager. Coordinate and build a strong work relationship with other departments.

ACCOUNTABILITY: Support Group/Quality Control Supervisor and Department Director

The ReConnect Program is designed to identify and reduce physical, mental, and environmental health challenges experienced by older adults age 60 and older in Orange County. The program offers short term early interventions including case management services, comprehensive psychosocial assessments, mental health screening, short term therapy, and linkages to needed community resources. In addition, the program offers educational, social, and therapeutic support groups throughout the county for participants to reinforce healthy life patterns and increase their quality of life.

POSITION OBJECTIVES: To conduct participant screening and intake process for enrollment. To cultivate relationship with referral sources. To manage clients and program data collection system to ensure timely and accurate data collection.

POSITION TITLE: REGISTERED NURSE

Concierge Care Navigators is a fee-for-service Geriatric Care Management program designed to eliminate fragmentation in care delivery. The Registered Nurse will advocate for older and disabled adults, providing short-term consultation and ongoing care management. Our services include evidence-based practices consisting of comprehensive assessments, coordination of care with physicians and health care providers, individualized online care plans with medical and non-medical goals, strategies for goal attainment, and linkages to supportive information and resources.

POSITION OBJECTIVE: To serve as trusted resource that creates a care plan linking client, family member, healthcare providers, physicians, community resources. Provide advice, insight and support while monitoring client cases in compliance with clinical standards.
